Due to the convenience of checkboxes embedded within the plan of care development of most electronic medical records, it is challenging for agencies to avoid the trap of mass-producing cookie-cutter POCs. In this session we will walk through the authoring of the POC, as well as discuss what needs to be done on it to meet accuracy and compliance standards. Value Based Purchasing full implementation began January 2023.  There is much to learn about the details of the outcome calculations.  This session will take you through the detail calculations of the VBP Model.  It will detail what we know to be the calculations of the TPS and the Linear Exchange Function for the nationwide rollout.  Lastly, agencies have received Pre-Implementation Performance Reports (PIPR) and this session will review how to evaluate the data and develop a plan for the agency to forge improvement.

  • Review the outcome measures included in HHVBP
  • Review the calculation methodology for Total Performance Score & Linear Exchange Function
  • Review PIPR and discuss corrective actions based on agency’s scores

The countdown to OASIS-E and home health value-based purchasing (HHVBP) is over. These high-impact changes became a reality on January1, 2023.During this session we will provide insights on how home health organizations transitioned fromOASIS-D1to OASIS-E. This major OASIS overhaul includes a new format, sections and scoring that will potentially impact agency quality performance and reimbursement.

  • What the increased focus on “social determinants of health” means for the future of HHAs and the reimbursement and quality measures.
  • How the OASIS-E expanded, and updated data elements will be used in the home health quality reporting program (HHQRP) measures.
  • Whether OASIS-E will impact star ratings and value-based purchasing (VBP).ยทHow HHAs can use the data being gathered by OASIS-E to improve care and position them for future success under forecastable CMS changes.

Medicare Advantage continues to expand its enrollment and all third-party payers continue to seek valuable care while keeping plan costs down.  It is more important than ever for providers to be able to successfully negotiate a partnership with these payers that is beneficial to all parties; the patient, the payer, and the provider.  Continued operational and financial success requires providers to leverage the quality care they provide to secure commensurate reimbursement. This session will identify the key elements in contracting with these payers.

  • Identify elements of payer contracts that should be negotiated, techniques to influence reimbursement/program savings and determine items that will potentially impact collections.
  • Provide practical approaches on how to leverage the agency’s strengths, care quality and leverage industry information.
  • Provide tools for contract management that may include example spreadsheets, tracking lists and software products. 
  • Review case examples of to approach negotiations, value-based tools to utilize and how to determine when to move forward or discontinue participating in a program.

Rehospitalizations are often preventable. While we recognize that acute care hospitalization (ACH) is a home care priority, a publicly reported outcome, and is a major impact in value-based purchasing, it is also important that we understand and appreciate the impact of the patient and family on repeated hospitalizations. Knowledgeable home care clinicians play a critical role in preventing rehospitalizations through understanding risk factors, disease management and patient/caregiver barriers to effective home management. To prevent ACH, home care clinicians must possess a high level of expertise in in assessing and monitoring for changes in condition and must intervene appropriately when early signs/symptoms are identified. The objectives of this session are:

  • Identify patient risk factors for ACH.
  • Examine evidence-based strategies to prevent ACH.
  • Determine when early signs/symptoms may be safely managed in the home.
